NOTE:

  1. Copies of the information being requested will be provided as soon as reasonably possible. The law provides 10 business days after receiving a written request to provide the record, deny the request, or notify the requester that it cannot immediately provide a response due to extraordinary circumstances.
  2. This request may be delayed if all the information is not provided.

PLEASE READ THE FOLLOWING BEFORE SIGNING THIS REQUEST

Your request for records will be processed in accordance with the requirements of the Government Records Access Management Act (GRAMA). 63-2-101 et. Seq. Utah Code. Your request will be handled as soon as reasonably possible, but may take up to ten business days to be granted.

The records that may be provided to you, subsequent to your request, may contain information that is classified as "Protected", and will be edited in accordance with GRAMA and may only be disclosed under certain circumstances. 63-2-2-2 (U.C.A.) . I understand that there is no charge to view a record.

I agree to pay a fee of $10.00 (per report) to cover the actual cost of research and duplicating a record if copies are requested. This fee is subject to change if extensive research and copies are requested.

NOTE: Utah Code Section 63G-2-206(1)(2)(3) states that a governmental entity may provide a record that is private, controlled, or protected to another governmental entity, a government-managed corporation, a political subdivision, the federal government, or another state. Further details are defined.

Description of Records Being Shared

NOTE: Utah Code Section 63G-2-206(4) says that before disclosing a record or record series to another governmental entity, another state, the United States, a foreign government... the originating governmental entity shall:

  1. inform the recipient of the record's classification and the accompanying restrictions on access; and
  2. if the recipient is not a governmental entity to which the Government Records Access and Management Act applies, obtain the recipient's written agreement that it will abide by specified access restrictions unless a statute, federal regulation, or interstate agreement otherwise governs the sharing of the record or record series.

Also, Utah code section 63G-2-206(6) states that an entity receiving a record is subject to the same restrictions on disclosure of the record as the originating entity.
